Full Payment

We ask that you pay in full for services that you receive at the time of your appointment. We accept Cash, Visa, Check, MasterCard, Discover and American Express.

Insurance Companies

Insurance companies have a contractual requirement that all co-payments are collected at the time of service. If you have questions about this process, just ask!

Deductible Policy

Effective December 1, 2013, for those who have a deductible policy, you will be required to pay a $50.00 up-front payment at the time of service. The additional payment will be left up to you to pay once your insurance processes the claim.

Pay Now

To keep our fees to a minimum, we ask that you pay your estimated liability at the time that you receive services. Statements are mailed out showing any remaining balance and may incur interest if they are not paid within 30 days. You are required to pay your balance in full unless special arrangements are made with our office. If you have a delinquent account, you must have approval from of our office before the next appointment can be made. Effective April 11th 2022, there will be a service charge of 3.99% of all credit/debit card processed. 

Labs and procedures are necessary at times for your child. It is possible that these services can be obtained at another outside facility, and you can expect to be billed from that facility as well. We ask that you get in touch with that facility if you have any questions regarding billing for that specific appointment.

The adult that brings the child in for their appointment is the person responsible for the payment at the time of service. A divorce decree is a legal agreement binding only two parties who made the agreement. Therefore, we are not involved in that contract. Both parties will still remain responsible for the child’s bills. The responsibility will be determined by which parent brings the child in at the time of service.

A $25.00 fee is issued for any returned checks. Missed or broken appointments that are not canceled 24-hours or more before the appointment time will also require a $25.00 fee.

If you have insurance, we can file your claims to the carrier as a courtesy. In the event that the insurance company decides not to cover your claim, you are responsible for the balance.

Our practice believes that this communication between the office and patient is extremely important. If you have any questions regarding your insurance explanation or its benefits, please contact your insurance company first. If you have any questions regarding your balance or you want to make a financial agreement, please talk to our office manager. We want to make sure any questions are clarified for your understanding.

Well Child Visits

All well child exams and physicals are performed by appointment only. Schedule your child’s exam in advance to make sure they stay on schedule. Please be aware that June, July and August are very busy due to back to school exams.

Sick Appointments

Our telephone lines open at 8:30 AM Monday through Friday. Please call as early as you can to ensure a same-day appointment for your child. In most cases, your appointment will be scheduled with the provider that is first available. Due to the personal, focused care that we want to provide for each child, walk-ins are very strongly discouraged.
